“Freedom of Information/Privacy Act
Request” Form G-639 (latest revision); then
complete and return it for action.

Per USCIS Form
G-639: “Any written format for Freedom of
Information or Privacy Act requests is
acceptable”. NOTE: If you use a
written format rather than Form G-639, be as specific
as you possibly can be with respect to names, dates,
places, etc.
If subject is
deceased, PROOF OF DEATH IS REQUIRED
(Obituary, Death Certificate, or other proof of
death).
